The effective date of the Lincoln Dental Care Good Patient RewardsSM Program is February 1, 2005, and only registrations entered on or after this date are eligible. The person who registers is the only eligible party to receive the reward and/or to designate the recipient of the reward. If the registrant departs from their position or responsibilities, the designated responsible party assumes responsibility.
The registrant may choose only one item from one of the rewards offered from Lincoln Dental Care per six month period upon completion of regular dental visit as long as patient is scheduled on or 30 days after 6-month recall date. If registrant has dental insurance, registrant is responsible for determining insurance coverage for each visit and must take this into consideration when scheduling their appointment.
The Rewards option must be requested in advance by registering for the program online, in-office, or over the phone. Upon completion of each regular dental visit that takes place exactly six calendar months after previous visit plus 30 days, the registrant must complete the Lincoln Dental Care Good Patient RewardsSM Fulfillment document indicating the reward choice. The Good Patient Rewards Fulfillment document must be signed by both a Lincoln Dental Care representative and registrant or designated responsible party.
A minimum of one general checkup visit and a six-month waiting period is required before claiming rewards. A general checkup visit is defined as any preventative services rendered during the visit to include teeth cleaning, x-rays, and comprehensive or periodic exams. Registrants are allowed to claim only two rewards per year beginning on the date of enrollment in the program. The reward will be awarded once all billing is calculated and paid in full. Rewards may not be used in combination with any other special offers, discounts or promotions.
Lincoln Dental Care has the right to change, limit, modify or cancel the Lincoln Dental Care Good Patient RewardsSM Program, its rules, regulations and rewards at any time, with or without notice, even though such changes may affect the value of, availability or the ability to obtain certain rewards. Lincoln Dental Care and its partners may, among other things: (a) withdraw, limit, modify or cancel any rewards; or (b) change program benefits, conditions of participation, or rules for earning or redeeming rewards. Lincoln Dental Care has the sole discretion to interpret and apply the Rewards program terms and conditions.
All terms and conditions control the redemption and distribution of the rewards earned.
